To send the samples for testing you need to get a ziplock bag for each material in question, number the bag and seal it along with the sample ( at least the size of the tea spoon or 1×1 inch).
All samples must be submitted with the proper completed Chain of Custody Form. COC form can be downloaded below. Please include the name of the owner, the sampling site (your address), the exact location in the house that the sample came from (example: basement, boiler room, etc…), and contact information (email and phone #).
Below are the necessary forms for submitting samples. These forms are Fillable PDFs which means they can be filled out on your computer after downloading the form. Once downloaded, open the form and fill out by clicking the proper fields and typing. When you are finished, you can print the form.
The samples, along with the written Chain of Custody can be mailed to 51 Gage Rd, East Brunswick, NJ 08816 or you can bring the sample yourself and leave it in the mail box. We will process it in the laboratory within 24 business hours of receiving the package and the written report will be emailed to you.
Samples can also be arranged to be picked up in New York City.
